Case-3343 Case-3343
This patient in her early 50s underwent revision breast augmentation with a simultaneous breast lift (augmentation mastopexy) performed by Dr. Geo N. Tabbal in Plano, Texas. She sought improvement in her upper pole fullness, sagging, and overall breast shape after two previous augmentations performed by other surgeons.
Her first augmentation, performed in the 1990s, involved placing implants above the chest muscle through incisions in the folds beneath the breasts (IMF). Approximately one year before her consultation with Dr. Tabbal, she underwent a second augmentation using incisions in the armpits to place implants below the muscle.
During her revision surgery, Dr. Tabbal identified “window shading” involving the lower portion of the pectoralis major chest muscle, which affected the relationship between the muscle and the implants. He repaired the muscle position and exchanged her ultra-high-profile implants for high-profile implants, placing the new implants beneath the muscle in the subpectoral position. A Wise-pattern breast lift, also called an anchor-pattern lift, was performed at the same time to address sagging and reshape the breasts.
The procedure was tailored to her concerns about upper breast fullness, breast position, and shape while addressing the muscle-related findings from her previous augmentation. These before-and-after photographs document her individual case. Results vary from patient to patient.
