This 53 year-old patient presented with a progressive deformity of her right breast approximately 10 years after a prior subglandular silicone breast augmentation. While she had done well for many years, she began to notice increasing firmness, discomfort, and visible distortion over the past 1–2 years. On examination, she was found to have a Grade III capsular contracture of the right breast, with superomedial displacement of the implant and associated asymmetry.

Surgical correction focused on both restoring comfort and improving breast aesthetics. The right breast underwent a total capsulectomy with removal of a Mentor™, 400 cc moderate-plus profile silicone implant, followed by placement of an Allergan™ 375 cc moderate profile, soft-touch implant in the subpectoral position. A matching implant exchange and pocket conversion was performed on the left side to optimize symmetry and balance.

This approach not only resolved the capsular contracture and associated pain, but also allowed for improved implant positioning, restoration of upper pole fullness, and a more natural breast shape. Overall, the patient achieved a softer feel, improved symmetry, and a more youthful contour.
